    Welcome home! Although we know you are glad to be out of the hospital, coming home from the hospital can be scary. Keeping track of information about your condition, medications, and your treatment plan may seem overwhelming. We are here to help.

    Our MyHeartVisit® Welcome Home Program provides you with weekly educational emails, weekly emotional support emails, and a trained peer-supporter who will call you every other week during a 12-week, post-hospitalization recovery period. And this program costs absolutely nothing for heart patients, parents and caregivers!

  • Educate Yourself

    An important step in becoming an empowered patient is to educate yourself about your condition. There is a lot of information out there, and it can be hard to find reliable information that is written for patients.

    Luckily, The Mended Hearts, Inc. is made up of patients like you, Our education and resources are created by patients and medical professionals and are easy to use.

  • Get Support

    The Mended Hearts, Inc. trains volunteers called Accredited Visitors who are patients and caregivers themselves to give emotional support by phone, email, or in-person (where available) to those who need it.

    Call or email us today, and we will connect you with a trained visitor. People who have been through their journey with heart disease can listen and answer questions from a patient perspective.
