Support & Education for People Living with Heart Failure
Empower HF is a comprehensive support and education program for people living with heart failure. This unique program will activate patients to learn more about heart failure, including their ejection fraction, educate patients through patient journey videos and educational resources, and connect patients to peer support, tools, and information to support them as they learn to live with and manage heart failure.
The Empower HF Program is carefully designed to support patients dealing with heart failure every step of the way. It provides a wide range of help, education, and resources that are personalized to fit each person's needs. The program is set up to help patients feel more empowered by first helping them recognize symptoms of heart failure and then encouraging them to understand a key measurement called ejection fraction.
